1990 – PARTIAL LAUNCH FAILURE: Intelsat 6 F-3, MM Commercial Titan III (CT-2), LC40, CCAFS – Second stage failed to separate from spacecraft due to sequence system error – Test Like You Fly, folks -MILESTONE: 35 years ago

1992 – LAUNCH: Galaxy 15, GD Atlas I, LC36B, CCAFS

1998 – PARTIAL LAUNCH FAILURE: LM AMC-14, Proton-M/Briz-M, LC200/39, Baikonur, Kazakhstan – early shutdown of Briz-M upper stage 

2023 – LM launches commercial ground control software for satellite constellations package using Horizon Command and Control and Compass Mission Planning Software

1963 – LAUNCH (3): Lockheed Polaris A2, SSBN600, ETR

The photo today is the fourth and last Commercial Titan launch, the launch of Mars Observer.  There are no other photos available of the commercial Titan launches that meet the stringent criteria that I use for photos in this group (public domain, government agency, proper attributions). 

On this day in 1990, CT-2 launched and payload separation was not achieved.  It’s a long story (and a completely preventable cause) and it has been discussed here previously.
